Family Nurse Practitioner - Comprehensive Primary Care Specialist

Brightwood Orthopedic and Family Medicine Care is seeking a dedicated Family Nurse Practitioner to join our team in Albany, New York. This full-time role offers an exciting opportunity to provide impactful primary care to our Medicare population. Work Environment

This role is situated in a collaborative and supportive environment where you will work onsite in home care settings. You will be part of a multidisciplinary team that includes physicians, nurse practitioners, physician assistants, medical assistants, and support from registered nurses and care managers. The collaborative care model is designed to allow healthcare providers to focus on what they do best—caring for patients—while working seamlessly with a network of elite specialists and hospitals for any specialty or acute care needs.

Key Responsibilities

Your day-to-day responsibilities will involve:

• Providing outstanding primary care services.
• Conducting thorough office visits for both routine and acute health issues.
• Administering annual wellness visits and comprehensive health risk assessments that highlight a holistic approach to patient health.
• Coordinating care with other healthcare providers, specialists, testing facilities, and external agencies to offer seamless patient care.
• Leading population health initiatives to ensure that all eligible patients receive timely and evidence-based preventive screenings, such as biennial breast cancer screenings.
• Managing phone triage and conducting outreach to patients in collaboration with the care team.
• Educating patients about their health conditions, care plans, and required treatments to enhance their understanding and engagement in their healthcare journey.
• Engaging in promotional activities to raise awareness about our services.
• Conducting home visits to ensure comprehensive patient care when needed.
• Reporting directly to the Medical Director while working closely with operational leadership to optimize patient care quality.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare, cultivating strong relationships that not only enhance their health but also promote their quality of life. Our care model is dedicated to maintaining the independence and well-being of our patients while enabling them to live life to the fullest, reflecting our commitment to addressing their unique healthcare needs.

Qualifications

The ideal candidate will have:

• A Master of Science in Nursing, Doctor of Nursing Practice, or have graduated from an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• An active DEA license.
• A solid foundation in primary care, particularly within internal medicine, geriatrics, or family medicine.

Preferred Experience

Preferred candidates will also demonstrate:

• A strong understanding of HCC (Hierarchical Condition Categories) documentation and ICD-10 coding practices.
• Experience with Health Risk Assessments (HRAs), enhancing overall patient care quality.
• A passion for teamwork and a commitment to effectively collaborate within multidisciplinary settings.

• Bilingual proficiency in applicable areas is a plus, allowing for enhanced communication with diverse patient populations.

Compensation and Benefits

The position offers a competitive salary range of USD 115,000 to 150,000 annually, commensurate with experience and expertise. In addition to a competitive salary, we provide:
