Work Environment

The work setting for this role is primarily onsite, located within the cozy and welcoming environment of our home care facility in Nassau. This allows for interactive engagement with patients and their families. Our full-time Nurse Practitioner can expect a work schedule generally between 9:00 a.m. and 6:00 p.m., with occasional evening hours until 8:00 p.m. Additionally, participation in local health fairs and outreach events during weekends may be required. A typical workweek will encompass approximately 40 hours, offering a well-rounded schedule that supports work-life balance.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, identifying both acute and chronic health issues.
• Diagnose and manage a diverse array of medical conditions, focusing on preventive care and health promotion to guide patients towards healthier lifestyles.
• Collaborate effectively with community organizations to design and implement impactful health education initiatives, enhancing community awareness on critical health issues.
• Provide culturally appropriate care that respects patients' backgrounds and health beliefs, advocating for their rights and access to necessary health services.
• Coordinate referrals and follow-up care to ensure seamless continuity of care, addressing the specific needs of each patient.

This role serves a diverse patient population, including families from various socio-economic backgrounds seeking comprehensive primary care services. As a Family Nurse Practitioner, you will deliver services that encompass wellness visits, immunizations, management of chronic diseases such as diabetes and hypertension, as well as acute illness evaluations. Qualifications

• Possession of a current Nurse Practitioner certification and an active state nursing license.
• A Master's degree in nursing from an accredited program is required.

Compensation and Benefits

Uprise Valley Pediatrics offers a competitive salary range of USD 120,000 to 220,000 annually, commensurate with experience and qualifications.
