Work Environment

This position is primarily onsite, working within a school environment where you will interact daily with students, families, and educational staff. Key Responsibilities

In your role as a Nurse Practitioner, you will undertake a variety of essential functions vital to our patients' healthcare journey. Key responsibilities include:

• Conducting comprehensive initial patient histories and performing thorough physical examinations to assess the well-being of students.
• Developing, implementing, and documenting personalized care plans, ensuring clear communication with patients and their families regarding treatment protocols and follow-up procedures.
• Performing daily patient rounds to monitor health progress, analyze vital signs, and make necessary adjustments to treatment regimens.
• Prescribing medications, ordering laboratory tests, and requesting relevant diagnostic studies, while evaluating results promptly to inform ongoing care strategies.
• Collaborating with attending physicians, nursing staff, and other healthcare professionals to provide a highly coordinated approach to patient care, addressing any inter-disciplinary concerns that arise.
• Preparing and maintaining accurate discharge summaries and medication prescriptions upon patient discharge, ensuring patients understand their post-discharge care instructions.
• Participating in continuous quality improvement initiatives, educational programs, and research activities that enhance patient care and contribute to professional development.

Patient Population / Client Focus

In this role, you will primarily focus on a pediatric population, engaging with students of various ages and backgrounds. The diverse needs of this demographic demand adaptability and an understanding of how health issues affect educational performance and overall well-being. You will work with families to address health concerns that may impede a student's ability to thrive academically and socially.

Qualifications

To be considered for this Nurse Practitioner position, candidates must possess:

• A Master’s Degree from an accredited Nurse Practitioner Program.
• National Board Certification in a relevant specialty area.
• A valid Nurse Practitioner and Registered Nurse license to practice in New York State.
• A collaborative practice agreement or attestation as mandated by the Nurse Practitioner Modernization Act of 2015.
• Current Basic Life Support (BLS) certification.

Preferred Experience

Ideal candidates will have previous experience working in a school-based or pediatrics setting, showcasing familiarity with issues related to adolescent health, chronic conditions, and preventative care. Experience in a collaborative practice model is a significant advantage, as is proficiency with electronic health record systems used in educational settings.
