Work Environment

The role is based onsite at a local school, providing you with the unique opportunity to work directly within the educational environment, connecting closely with families and children. Our working hours typically range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, you may participate in weekend community health outreach events or health fairs, actively engaging with community members beyond regular office hours. Expect an average workweek of approximately 40 hours that allows for a balanced workload while making a meaningful impact.

Key Responsibilities

• Conduct comprehensive health assessments, screenings, and wellness checks for children and their families, ensuring early detection and intervention for health issues.
• Diagnose and manage both acute and chronic medical conditions, providing essential preventive care and promotion of healthy lifestyles.
• Collaborate with local community organizations and stakeholders to design and implement health education initiatives tailored to family needs.
• Deliver culturally sensitive care and serve as an advocate for patients’ rights, fostering access to healthcare and necessary services.
• Coordinate referrals and follow-up care to ensure patients receive appropriate and ongoing medical attention.
• Maintain detailed medical records in compliance with state and federal regulatory standards, ensuring patient confidentiality and care continuity.
• Engage in community outreach programs and health fairs, promoting awareness and delivering essential health screenings.
• Stay informed about current community health trends, evidence-based practices, and evolving healthcare policies that affect families.

Patient Population / Client Focus

You will be working primarily with children and their families, addressing diverse healthcare needs in a school setting. This population includes underserved communities who may face barriers to healthcare access. By fostering strong relationships, you will not only assess and manage health conditions but also provide health education that empowers families to take charge of their wellbeing.

Qualifications

• Current certification as a Nurse Practitioner and an active nursing license in the state of New York.
• A Master’s degree in Nursing from an accredited institution.
• A minimum of three years of clinical experience in nursing, ideally with a focus in community or public health settings.
• Strong interpersonal and communication skills to effectively engage with a diverse patient population.

Compensation and Benefits

• This position offers a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
