Work Environment

Your position will be onsite at our Assisted Living Facility, where you will work closely with residents and their families in a supportive and collaborative environment. Our team fosters a culture of continuous learning and encourages participation in community outreach, ensuring that you’re not only treating patients but also engaging with the community to improve overall health literacy.

The typical work schedule for this role is from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m., ensuring accessibility for all patients. Participation in community events or health fairs during weekends may also be necessary, contributing to our mission of community health enhancement.

Key Responsibilities

• Conduct comprehensive health assessments and screenings to identify health concerns for both individuals and families in the community.
• Diagnose and manage a range of acute and chronic health conditions, emphasizing preventive care and health promotion strategies.
• Collaborate with community organizations and local health departments to develop and implement targeted health education programs, addressing specific needs in the population.
• Provide culturally sensitive care, advocating for patients' rights and assisting them in navigating healthcare services.
• Coordinate referrals and follow-up care with specialists to ensure a seamless continuum of care for patients.
• Maintain accurate and detailed medical records in compliance with all regulatory standards and best practices.
• Participate actively in community outreach events and health fairs, facilitating health screenings and educational workshops to promote community wellness.
• Stay current with community health trends, emerging evidence-based practices, and relevant healthcare policies to inform your practice.

Qualifications

• Current certification as a Family Nurse Practitioner.
• An active nursing license in the state of New York.
• A Master’s degree in Nursing from an accredited institution is required.

• A minimum of 3-5 years of clinical experience, preferably in diverse community settings.

Compensation and Benefits
