Key Responsibilities

• Conduct thorough cardiovascular assessments through detailed history taking and advanced physical examinations to identify patients' needs.
• Develop comprehensive treatment plans and manage ongoing care for patients with heart conditions, including hypertension, heart failure, and arrhythmias.
• Educate patients and their families on heart disease management, lifestyle modifications, medication adherence, and preventive health measures, fostering health literacy within the community.
• Monitor and document patient progress meticulously, utilizing electronic health records to adjust treatment plans and ensure optimal patient outcomes.
• Participate actively in follow-up appointments and discharge planning, ensuring a seamless transition from inpatient to outpatient care while addressing any barriers patients may face.
• Stay informed on emerging trends in cardiology and integrate evidence-based practices into daily routines to enhance care quality.

Qualifications

• A Master's degree in Nursing from an accredited institution.
• Current New York State licensure as a Nurse Practitioner (NP) is required.

Preferred Experience

Compensation and Benefits

The salary for this full-time position ranges from USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
