Key Responsibilities

Patient Population / Client Focus

Your primary focus will be on adults and seniors within the Assisted Living Facility, where you will address their unique physical and emotional healthcare needs. Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited institution is required.
• Active certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized bodies such as the ANCC or AANP.
• A current, unrestricted Nurse Practitioner (APRN) license to practice in New York State.
• Possession of an active DEA number and NPI number is essential.
• Minimum of 2 years of relevant clinical experience, particularly in settings focused on primary care or family medicine.
• Demonstrated clinical assessment and diagnostic skills with an emphasis on independent practice.
• Exceptional communication and interpersonal skills, facilitating productive relationships with diverse patient populations.
• Proficiency in electronic health record (EHR) systems to document and manage patient information efficiently.

Preferred Experience

• At least 2-5 years of clinical practice experience in community health or public health settings, enhancing your capability to address diverse healthcare needs.
