Work Environment

The NP will work onsite at our outpatient office in South Bethlehem, where a collaborative and supportive team-oriented environment awaits. In this role, you will work closely with local clinical leadership and interdisciplinary teams to ensure the highest standards of care are met. The position requires in-person patient interactions to provide adequate face-to-face coverage, ensuring that our facilities meet the resident care needs promptly.

Key Responsibilities

• Collaborate with clinical leadership and local operations to achieve the organization’s strategic goals and enhance patient outcomes.
• Support initiatives focused on value-based care, ensuring patients receive the most appropriate and effective treatments.
• Deliver exceptional customer service, actively engaging with patients and their families to foster satisfaction and retention.
• Operate ethically within the bounds of the Scope of Practice and Board Certification, adhering to all state and federal regulations.
• Facilitate coordinated care between patients, primary care physicians, and specialists—including telemedicine providers—ensuring seamless transitions of care.
• Conduct comprehensive assessments, diagnose conditions, and create personalized treatment plans for both chronic and acute health issues.
• Order and interpret diagnostic tests, adjusting treatment plans in response to findings to optimize patient health outcomes.
• Engage in family conferences and provide education on advanced care planning, tailoring discussions to meet individual patient needs.
• Maintain meticulous documentation within our Electronic Medical Record (EMR) system, ensuring compliance and accuracy.

Patient Population / Client Focus

The Nurse Practitioner will primarily focus on serving senior patients and those from low-income backgrounds, addressing their unique healthcare needs with empathy and a thorough understanding of the challenges they face.

Qualifications

• U.S. work authorization.
• Master of Science in Nursing, Doctor of Nursing Practice, or successful completion of an NP Certificate Program.
• National certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Active, unrestricted state license to practice as a Nurse Practitioner.
• Current DEA license for prescribing medications.

Preferred Experience

• Prior experience working in outpatient settings, particularly with elderly or underserved populations.
• Familiarity with electronic medical records (EMR) systems to streamline patient documentation and care coordination.
• Proven ability to work collaboratively within a multidisciplinary healthcare team.

Compensation and Benefits
