Key Responsibilities

• Conduct thorough physical examinations, including a comprehensive assessment of patients' health history.
• Diagnose and manage acute and chronic illness cases, utilizing evidence-based practice to inform clinical decision-making.
• Evaluate diagnostic tests and imaging results to determine appropriate treatment plans.
• Prescribe and manage medications, ensuring adherence to best practices in pharmacology and safety.
• Develop, implement, and modify care strategies in alignment with patients' individual needs, preferences, and health goals.
• Facilitate timely referrals to specialists and coordinate with other healthcare providers to promote comprehensive patient care.
• Engage in healthcare education with patients to promote understanding and adherence to health management plans.
• Document patient care activities accurately and timely in our electronic health record (EHR) system.
• Collaborate effectively with physicians, nursing staff, and administrative personnel to enhance patient experience and clinic workflow.

Patient Population / Client Focus

As a Nurse Practitioner at Traditions Medical Health Clinic, you will serve a diverse patient population ranging from children to seniors, focusing on both urgent care needs and ongoing health management. Qualifications

• Master's degree in Nursing and Nurse Practitioner program from an accredited institution.
• Active Nurse Practitioner license in New York State.
• At least 2 years of clinical experience as a Nurse Practitioner in an urgent care or similar setting.
• Nationally certified Nurse Practitioner with prescriptive authority.
• Extensive knowledge of medical terminology and pharmacology.
• Exceptional communication and interpersonal skills, with the ability to maintain professionalism in various settings.

Preferred Experience

Compensation and Benefits

• Annual salary range: USD 150,000 – 160,000 , commensurate with experience and qualifications.
