Work Environment

As an Internal Medicine Nurse Practitioner, you will work primarily in an onsite home care setting, fostering in-depth relationships with patients who may have complex medical needs. Key Responsibilities

• Deliver consistent, high-quality healthcare to patients in compliance with established national and community standards.
• Conduct thorough assessments of patient health data, including vital signs and medical histories, to establish effective care plans.
• Collaborate closely with office staff to optimize clinic workflows, ensuring efficiency in patient scheduling and information management.
• Serve as a liaison between practice management, hospital administration, and staff, facilitating communication and workflow improvements.
• Support regulatory compliance initiatives by maintaining up-to-date knowledge of healthcare regulations and associated best practices.
• Actively engage in patient management and serve as a role model for best practices aligning with health system goals.
• Document comprehensive patient care records within a two-business-day timeframe, ensuring accuracy and clarity in all communications.
• Stay informed on developments in clinical practice and be proactive in sharing insights with the healthcare team.

Patient Population / Client Focus

This role primarily serves adult and geriatric patients. The Nurse Practitioner will work closely with individuals presenting with chronic illnesses, acute health issues, and preventive care needs. Your ability to interpret age-specific health requirements and provide tailored interventions will be essential in supporting patients’ diverse healthcare journeys.

Qualifications

• Current NYS certification/registration as a Family, Adult, or Pediatric Nurse Practitioner is required.
• AANP Board Certification and an active DEA license are mandatory.
• Must be able to gain appropriate privileges at F. F. Thompson Health.
• Knowledge of ICD10 coding is preferred but not essential.

Preferred Experience

• Previous experience as a Nurse Practitioner, particularly in an outpatient clinical medicine setting, is highly desirable.

Compensation and Benefits

The compensation for this position ranges from USD 150,000 to 160,000 annually, reflecting your skills and experience within this essential role.
