Work Environment

This full-time role is onsite, allowing you to engage directly with patients and their families in a home care setting. Key Responsibilities

• Conduct thorough health assessments and detailed physical examinations to accurately diagnose and develop individualized treatment plans for patients.
• Order, interpret, and discuss diagnostic tests, including lab work and imaging studies, to ensure timely and informed decision-making.
• Prescribe medications and implement therapeutic interventions tailored to patient needs within regulatory guidelines.
• Educate patients and their families on prevention strategies, health promotion, and the management of chronic conditions, empowering them to take an active role in their health.
• Collaborate with a multidisciplinary team to streamline care coordination, manage referrals, and facilitate follow-up, ensuring continuity and quality of care.
• Document patient interactions meticulously in electronic medical records (EMR), adhering to best practices and regulatory standards.
• Stay updated on clinical guidelines, healthcare policies, and new medical technologies to enhance service delivery.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited program.
• Current licensure as a Nurse Practitioner in New York.

• Proven clinical assessment and diagnostic skills, with the ability to make sound, evidence-based decisions.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 160,000 annually, complemented by a comprehensive benefits package.
