Work Environment

This position is primarily onsite, as our Nurse Practitioners will conduct patient evaluations and follow-up visits in various home environments. The role demands a flexible approach, allowing NPs to adapt their schedules to meet the unique needs of our patients. Key Responsibilities

• Conduct thorough and comprehensive physical examinations of patients to assess their health and medical needs.
• Develop, implement, and periodically revise patient management policies and procedures tailored to individual needs.
• Facilitate referrals to other healthcare professionals, specialists, and medical facilities as necessary for continued patient care.
• Maintain clear and effective communication with collaborating physicians or specialists regarding the ongoing care of our patients.
• Coordinate and communicate patient care activities to clinical staff to ensure consistency and clarity in treatment plans.
• Perform administrative tasks as required, contributing to a seamless operational workflow.

Patient Population / Client Focus

Our patient population primarily includes individuals who require home-based healthcare services. This may encompass a diverse range of conditions, from chronic illnesses to post-surgical recoveries. The Nurse Practitioner will work closely with patients and their families, creating personalized care plans that prioritize safety, comfort, and quality of life. Emphasis is placed on understanding each patient’s unique situation, allowing for tailored healthcare solutions that enhance overall wellbeing.

Qualifications

• Possession of a valid national certification from either AANP or ANCC.
• An active, unrestricted Nurse Practitioner license in the state of New York is mandatory.
• Authorization to work in the U.S. is required.
• The candidate should possess a Master of Science in Nursing, Doctor of Nursing Practice, or have graduated from an accredited NP Certificate Program.
• A capacity to work in a dynamic and adaptable environment is essential, as needs may change based on patient circumstances.

Preferred Experience

While prior experience in home healthcare is advantageous, we value a diverse range of clinical backgrounds. Candidates who have experience working in outpatient settings or similar environments, and those who have demonstrated strong clinical judgement in autonomous roles, will be given preference. Experience in patient education and holistic health approaches will also be beneficial in this role.

Compensation and Benefits

We offer a competitive annual salary range of USD 150,000 to 160,000 , commensurate with experience and caseload.
