Work Environment

This position is primarily onsite, with Nurse Practitioners visiting patients in their homes to provide hands-on care. Key Responsibilities

• Conduct thorough physical examinations and comprehensive health assessments to evaluate patients' conditions and healthcare needs.
• Diagnose and treat both acute and chronic illnesses, ensuring an evidence-based approach to patient care that aligns with the latest medical guidelines.
• Prescribe appropriate medications and develop customized treatment plans that take into account patients’ medical histories and preferences.
• Administer immunizations and vaccinations in accordance with local public health guidelines to encourage community health.
• Provide detailed counseling on health promotion, disease prevention strategies, and lifestyle modifications, aiming to empower patients in managing their health.
• Order and interpret diagnostic tests, communicating results effectively to patients and incorporating findings into care plans.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive and coordinated patient care.
• Maintain accurate and up-to-date medical records, utilizing our EHR system to document patient encounters and treatment progresses.

Patient Population / Client Focus

You will serve a diverse patient population, treating individuals ranging from pediatrics to geriatrics. Qualifications

• Education: A Master’s degree in Nursing from an accredited institution is required.
• Licensing: A current, active Nurse Practitioner license in New York state.
• Certification: Valid certification as a Family Nurse Practitioner is mandatory.

Preferred Experience

• A minimum of 2 years of clinical experience in a similar role is required, with preference given to those who have worked in home care settings.

Compensation and Benefits

We offer a competitive salary ranging from $150,000 to $160,000 annually, commensurate with your level of experience and dedication to patient care.
