Work Environment

Key Responsibilities

• Conduct thorough patient assessments, including taking initial histories and performing detailed physical examinations.
• Develop, document, and communicate individualized plans of care, including necessary follow-up to ensure comprehensive screening and preparation of patients.
• Perform daily patient rounds to review progress, address concerns and provide updates to patients and families.
• Order appropriate medications, laboratory tests, and diagnostic procedures while ensuring all orders comply with established protocols.
• Interpret laboratory results and communicate findings to patients and interdisciplinary team members.
• Collaborate with attending physicians, residents, nursing staff, and other healthcare providers to foster a multidisciplinary approach to care.
• Document discharge summaries, prescriptions, and referrals accurately while providing patients with clear post-discharge care instructions.
• Fulfill additional duties as required, all of which are essential functions of the role as per the Americans with Disabilities Act.

Patient Population / Client Focus

In this role, you will care for a diverse patient population within the home care setting, focusing primarily on individuals requiring ongoing internal medicine support. Many patients may present with chronic conditions or complex healthcare needs, making your role pivotal in managing treatment plans and improving patient care outcomes. Qualifications

• A Master’s Degree from an accredited Nurse Practitioner Program is required.
• National Board Certification in your specialty area, along with a valid Nurse Practitioner license to practice in New York State, is necessary.
• A current Registered Professional Nurse license in New York State is also required.
• Either a collaborative practice agreement with designated protocols filed with the New York State Department of Education or an attestation of collaborative relationship per the Nurse Practitioner Modernization Act of 2015 is needed.
• Current Basic Life Support (BLS) certification is mandatory.

Preferred Experience

• Prior experience in internal medicine or home care settings is highly desirable.
• Proficiency in electronic health record systems and telehealth platforms preferred.
• Experience in coordinating multidisciplinary care for patients with chronic medical conditions.
• Strong communication skills to effectively educate patients and families regarding health management.

Compensation and Benefits

The salary for this position ranges from USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
