Work Environment

This position is located onsite, providing home care services directly to patients within their living environments. Our Nurse Practitioner will engage in meaningful interactions with clients, tailoring care plans to fit individual health needs. Core working hours are between 9:00 a.m. and 6:00 p.m., with occasional evening clinics and weekend community outreach initiatives. This schedule ensures a balanced work-life integration while allowing for active participation in community health programs.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, utilizing a patient-centered approach to identify health concerns.
• Diagnose and manage acute and chronic medical conditions, emphasizing preventive care and health promotion strategies.
• Design and implement health education programs in collaboration with community organizations, focusing on disease prevention and wellness.
• Ensure culturally sensitive care, advocating for patients' rights, and increasing access to healthcare services for underserved populations.
• Coordinate referrals and follow-up care, maintaining continuity and quality of care for patients within the community.
• Document patient interactions meticulously in compliance with regulatory standards, ensuring accurate and comprehensive medical records.
• Engage in community outreach events and health fairs, providing valuable health screenings and educational resources to the public.
• Stay informed on community health trends and evidence-based practices to enhance service delivery and patient outcomes.

Patient Population / Client Focus

This role will focus on serving a diverse patient population, particularly those who are underserved or lacking adequate healthcare access. You will be actively involved in addressing a variety of health issues, spanning from routine preventive care to management of chronic conditions. This community-driven approach ensures that all patients receive the highest standard of care, tailored to their unique needs.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current and unrestricted Nurse Practitioner (APRN) license in New York State.
• DEA# and NPI# are required to prescribe medications.
• Strong clinical assessment and diagnostic skills, complemented by excellent communication and interpersonal abilities.
• Competence in utilizing electronic health records (EHR) systems to document patient interactions and treatment plans.

Preferred Experience

• A minimum of three years of clinical experience, preferably in primary care or family medicine and within community health or public health settings.
• Proven ability to work both independently and collaboratively as part of a healthcare team.
• Experience in providing care to diverse populations, demonstrating cultural competency and sensitivity.

Compensation and Benefits

The position offers a competitive salary range of $120,000 to $130,000 per year, subject to qualifications and experience.
