Work Environment

The role is onsite, with a strong focus on home care, allowing for direct patient interactions in their everyday environments. Our facility supports a vibrant and collegial atmosphere, with resources available to facilitate teamwork among NPs, PAs, and physicians. Your ability to build rapport with patients and collaborate with fellow healthcare providers is essential for success in this position.

Key Responsibilities

• Conduct comprehensive health assessments through interviewing patients and performing thorough physical examinations.
• Obtain, update, and analyze medical histories to create tailored care plans that meet the individual needs of our patients.
• Collaborate closely with physicians to address both short-term and long-term patient care issues, ensuring all clinical decisions are made in the best interest of the patient.
• Develop, implement, and adjust patient management plans based on ongoing assessments and feedback from patients and their families.
• Participate in team meetings to discuss patient progress, share insights, and implement best practices for quality improvement in care delivery.
• Engage in patient education to empower individuals and their families about health conditions, treatment options, and preventive measures.
• Document clinical findings and patient interactions meticulously to maintain accurate records and compliance with healthcare regulations.

Patient Population / Client Focus

You will be working primarily with adult patients, focusing on those with chronic conditions related to internal medicine. This includes individuals with diabetes, cardiovascular issues, respiratory diseases, and other complex health challenges that require ongoing management. It is essential that you foster a trusting relationship with patients and provide them with personalized care tailored to their unique needs.

Qualifications

• Bachelor's Degree in Nursing or a related field from an accredited program.
• Current Nurse Practitioner or Physician Assistant licensure in the state of New York is required.
• Completion of the Physician Assistant National Certifying Examination for PAs or an equivalent certification for NPs.

Preferred Experience

• A minimum of two years of experience in an ambulatory care setting, with a strong emphasis on internal medicine.
• Familiarity with home care protocols and patient management strategies that align with home-based care settings.

Compensation and Benefits

This full-time role offers a competitive salary range of USD 150,000 to 160,000 annually, commensurate with experience and qualifications. In addition to a base salary, we provide an attractive benefits package that includes:
