Position Overview

SLI Rehab & Physical Therapy is seeking a dedicated Family Nurse Practitioner (FNP) to join our compassionate healthcare team in Delmar, New York. Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, ensuring a comprehensive understanding of health histories and risk factors.
• Diagnose and manage both acute and chronic medical conditions while developing individualized treatment plans that include preventive care and health promotion strategies.
• Implement health education programs in collaboration with community organizations to empower individuals with knowledge and resources for healthier living.
• Provide culturally sensitive care, effectively advocating for patients' rights and access to necessary healthcare services, reflecting our commitment to health equity.
• Coordinate referrals and follow-up care required to maintain continuity, forging effective connections between patients and the healthcare services they need.
• Maintain accurate medical records utilizing our electronic health record (EHR) system, ensuring compliance with all regulatory standards and facilitating effective communication among healthcare team members.
• Engage in community outreach efforts, including participation in health fairs and other events, promoting health awareness and providing free health screenings.
• Stay informed about emerging trends in community health, adapting best practices to continually enhance service delivery.

Patient Population / Client Focus

Your work will focus on a diverse patient population, including families from varied socioeconomic backgrounds. You will be instrumental in delivering care to underserved communities, understanding the unique challenges they face, and providing tailored solutions that support their health and wellness. Your holistic approach will allow you to foster meaningful relationships with patients, enhancing trust and improving healthcare adherence.

Qualifications

• Current certification as a Nurse Practitioner (NP) and an active state nursing license in New York.
• A Master's degree in Nursing from an accredited institution, with emphasis on family practice.
• A strong background in preventative and community health practices.
• Demonstrated commitment to social justice and advocacy for vulnerable populations.
• Excellent communication and interpersonal skills essential for engaging with a diverse clientele.

Preferred Experience

• 3-5 years of relevant clinical experience, particularly in community health or public health settings, is highly desirable.
• Experience developing and implementing health education initiatives or community outreach programs.
• Familiarity with EHR systems and a solid understanding of healthcare documentation standards.

Compensation and Benefits

• Competitive annual salary ranging from $180,000 to $200,000, commensurate with experience and qualifications.
