Work Environment

Located in the heart of Troy, New York, our assisted living facility is designed to support residents in maintaining their independence while receiving the necessary healthcare. As a Family Nurse Practitioner, you will work closely with a multidisciplinary team, including physicians, nurses, and allied health professionals. Key Responsibilities

• Conduct thorough patient assessments, including initial screenings and comprehensive health history evaluations.
• Perform physical examinations for residents and document findings accurately, ensuring compliance with healthcare regulations and protocols.
• Analyze diagnostic test results to identify patient health concerns and develop tailored treatment plans.
• Monitor patient progress through follow-up appointments, ensuring that treatment plans are effective and making adjustments as necessary.
• Communicate clearly with residents and their families, providing education on health maintenance, chronic disease management, and preventive care.
• Implement harm reduction strategies to support patients facing critical health challenges, enhancing their overall quality of life.
• Maintain accurate and up-to-date patient records in our electronic health system, monitoring productivity metrics aligned with organizational goals.

Patient Population / Client Focus

The ideal candidate will work with a varied patient population, primarily older adults residing in an assisted living setting. This role provides an opportunity to form meaningful connections with patients, understanding their unique healthcare needs and delivering compassionate care.

Qualifications

• Certification as a Family Nurse Practitioner (FNP) in Family Medicine.
• Current, valid New York State Family Nurse Practitioner license.
• A Bachelor's degree in Nursing and a Master's degree from an accredited FNP program, or completion of an approved supplemental education program.
• Minimum of two years of clinical nursing experience, preferably in primary care or geriatric settings.
• Proven ability to perform physical assessments, diagnose common illnesses, and manage chronic diseases effectively.
• Strong communication skills, enabling effective interaction with a diverse patient base and interdisciplinary team.

Preferred Experience

• A strong background in geriatric care or experience working within assisted living facilities.
• Familiarity with electronic health record (EHR) systems and telehealth integration to improve patient access to care.

Compensation and Benefits

This position offers a competitive salary range of USD 150,000 to 160,000 per year, reflective of your experience and expertise.
