Key Responsibilities

• Perform comprehensive health assessments that include evaluations, screenings, and family health history reviews, developing treatment plans tailored to individual needs.
• Diagnose and manage both acute and chronic conditions through evidence-based practices, ensuring timely preventive care and health promotion efforts.
• Collaborate actively with community organizations, health professionals, and stakeholders to create and implement health education programs addressing local public health issues.
• Promote culturally sensitive practices, advocating for patient rights and resources that enhance access to healthcare services, particularly for underserved populations.
• Manage patient referrals and follow-up processes efficiently, ensuring continuity of care that addresses long-term wellness goals.
• Maintain meticulous medical records in compliance with healthcare regulations, utilizing electronic health record (EHR) systems for efficient documentation and information sharing.
• Engage in community outreach events, providing health screenings and education at health fairs to inform and empower individuals in the Troy area.
• Stay informed about latest community health trends, best practices, and policies impacting patient care and public health initiatives.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A Master’s degree in Nursing from an accredited program is required.

• Strong interpersonal and communication skills to effectively connect with and serve a diverse patient population.

Compensation and Benefits
