Work Environment

This position is primarily onsite and involves traveling to patients' homes within the Troy, New York area. Our work environment is dynamic, embracing a collaborative spirit among healthcare professionals. You will be working closely with a multidisciplinary team to address the unique health challenges faced by our patient population. In addition to providing direct care, you will engage in health education initiatives, community outreach programs, and health fairs on weekends.

Key Responsibilities

• Conduct comprehensive health assessments, including physical examinations and preventive screenings for patients at their residences.
• Diagnose and manage a variety of acute and chronic conditions, emphasizing preventive care and health promotion tailored to individual patient needs.
• Collaborate with local health organizations to create and implement impactful health education programs that foster wellness in the community.
• Advocate for patients’ rights and equitable access to healthcare services, ensuring culturally sensitive care is provided at all times.
• Coordinate patient referrals and follow-up care, ensuring continuity in treatment and support.
• Maintain accurate and detailed medical records in accordance with regulatory standards to facilitate optimal patient care.
• Participate in community health fairs and events, providing valuable health screenings and raising awareness about health resources.
• Stay informed on current community health trends, best practices, and health policies to enhance care delivery.

Patient Population / Client Focus

This role involves working closely with a diverse population, including individuals who may have varying access to healthcare resources. You will advocate for and provide care to underserved groups, ensuring that your approach is respectful and responsive to the cultural contexts of your patients. The goal is to empower patients to take an active role in their health management and promote healthy lifestyle choices.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized bodies such as ANCC or AANP.
• Current, unrestricted Nurse Practitioner (APRN) license in New York State along with a valid DEA# and NPI#.
• Strong clinical assessment and diagnostic skills, enabling effective patient evaluations and management.
• Exceptional communication and interpersonal skills, capable of establishing rapport with patients and families from diverse backgrounds.
• Proficiency in electronic health record (EHR) systems for documentation and coordination of care.

Preferred Experience

• A minimum of 3-5 years in clinical practice, particularly in primary care or community health settings.
• Experience in providing health education and outreach programs is highly desirable.
• A solid understanding of the unique challenges and health needs of the local population served.

Compensation and Benefits

• Competitive salary ranging from USD 150,000 to 160,000 annually.
