Work Environment

This onsite position is located within an Assisted Living Facility, imparting a unique blend of acute care and community healthcare. Key Responsibilities

• Conduct comprehensive assessments of patients, effectively managing both physical and psychological health issues through a balanced approach.
• Analyze and interpret both normal and abnormal clinical findings, developing tailored treatment plans that address the individual needs of each patient.
• Continuously evaluate the effectiveness of treatment regimens and make necessary adjustments based on clinical outcomes and patient feedback.
• Prescribe appropriate medications and immunizations, adhering to contemporary clinical guidelines and protocols.
• Maintain clear communication channels with fellow healthcare providers, ensuring a seamless continuum of care through collaboration and information sharing.
• Document all patient interactions with a focus on accuracy, completeness, and compliance with our healthcare standards.
• Participate in the development of clinical policies and procedures that enhance our service quality.

Patient Population / Client Focus

You will primarily engage with patients in an Assisted Living Facility, focusing on older adults who often present with complex, multifactorial health issues. As a Nurse Practitioner, you will play a pivotal role in assessing and managing chronic pain and cardiovascular conditions, all while taking into consideration the unique needs that arise from this demographic.

Qualifications

• A Master's degree in Nursing from an accredited Nurse Practitioner program.
• A valid Registered Nurse (RN) License issued by the State of New York.
• Basic Life Support (BLS) Certification required.
• Strong interpersonal and communication skills, enabling effective collaboration within a diverse healthcare setting.

Preferred Experience

• At least 2-3 years of experience in cardiology or a related field is preferred.
• Experience with pain management or geriatric care is highly advantageous.

Compensation and Benefits
