Key Responsibilities

• Collaborate effectively with physicians and interdisciplinary health care teams to ensure comprehensive care for patients.
• Perform thorough assessments by obtaining detailed patient medical, physical, and psychosocial histories, employing a holistic approach to health care.
• Conduct preventive screening procedures and health assessments that are age-appropriate, ensuring early intervention and preventive health measures.
• Utilize clinical findings and patient history to formulate differential diagnoses and establish treatment plans.
• Educate patients and their families on health management strategies, medications, and follow-up care, empowering them in their health journeys.
• Prescribe medications and implement therapeutic interventions, both pharmacologic and non-pharmacologic, tailored to individual patient needs.
• Document patient care using electronic health records (EHR) systems, ensuring accuracy and compliance with regulatory standards.

Qualifications

• Master’s Degree as a Nurse Practitioner.
• Active Registered Nurse (RN) licensure in the State of New York.
• Minimum of 3 years of relevant clinical experience in an internal medicine or primary care setting.
• Proficient in utilizing medical technologies and electronic health records (EHR) for documentation and patient management.
• Strong interpersonal skills with a commitment to educating and advocating for patients' needs.

Preferred Experience

Compensation and Benefits
