Radiant Wellness Clinic is seeking an experienced and compassionate Family Nurse Practitioner (FNP) to join our dynamic team in Troy, New York. This full-time position focuses on delivering high-quality, personalized healthcare through a concierge model that includes house calls and community engagement. Work Environment

This position is based at our state-of-the-art facility in Troy, NY, where we emphasize a supportive and collaborative work culture. The majority of care will be provided onsite, and you will also conduct house calls to accommodate patients who may have difficulty accessing traditional healthcare settings. The typical workweek is approximately 40 hours, with a flexible schedule from 9:00 a.m. to 6:00 p.m. Occasionally, you may be required to work evening clinics until 8:00 p.m. and participate in community outreach events during weekends.

Key Responsibilities

• Conduct thorough health assessments, including physical examinations and screenings for individuals and families, utilizing evidence-based practices.
• Diagnose, manage, and provide treatment plans for acute and chronic medical conditions, focusing on preventive care and health promotion strategies.
• Develop and implement educational health programs alongside community organizations, addressing specific health challenges faced by our patients.
• Provide culturally sensitive care by understanding and considering the diverse backgrounds and unique needs of each patient.
• Advocate for patient rights and work to enhance access to healthcare services, coordinating referrals and follow-up care as necessary to ensure continuity.
• Document patient information comprehensively and accurately in compliance with healthcare regulations, ensuring continuity in patient care.
• Engage in community outreach events and health fairs, providing screenings and services to promote health awareness.
• Stay abreast of current trends in community health, emerging evidence-based practices, and relevant health policies impacting your patients.

Patient Population / Client Focus

Your primary focus will be on serving individuals and families within the Troy community, particularly those from underserved populations who may have limited access to traditional healthcare resources. You will engage with patients of various ages and backgrounds, addressing their diverse healthcare needs while advocating for health equity.

Qualifications

• Possess a valid and current Family Nurse Practitioner certification.
• Hold an active state nursing license in New York.
• Possess a Master's degree in Nursing from an accredited institution.
• A minimum of 3 years of clinical experience, preferably in community health or public health settings, is essential.

Preferred Experience

• Experience in concierge medicine or house call services is highly valued.
• A proven track record of participation in community health initiatives or public health education.
• Strong interpersonal skills with the ability to communicate effectively and empathetically with a diverse patient population.

Compensation and Benefits

This position offers a competitive salary range of USD 180,000 – 200,000 per year, commensurate with experience and qualifications.
