Primary Care Nurse Practitioner - Full-Time, Onsite Position

Position Overview

The Integrative Health and Wellness Clinic is actively seeking a dedicated Nurse Practitioner (NP) to join our team in Troy, New York. This full-time, onsite role will be based within an Assisted Living Facility, allowing you to provide essential healthcare services to a dynamic population in need of comprehensive care. As a critical member of our healthcare team, you will work under the supervision of the Chief Nurse Practitioner and in collaboration with physicians, fellow NPs, nurses, and other essential support staff.

Work Environment

Your work will primarily take place in an Assisted Living Facility environment, ensuring you can provide immediate and ongoing care to residents who require a mix of routine assessments and chronic disease management. Key Responsibilities

As a Nurse Practitioner with our team, you will:

• Conduct thorough patient assessments by obtaining medical histories and performing both comprehensive and focused physical examinations.
• Order, interpret, and analyze diagnostic tests to create and adjust treatment plans for both acute and chronic conditions.
• Work autonomously as well as collaboratively to deliver high-quality care tailored to the unique needs of each patient and their specific health concerns.
• Maintain meticulous documentation of patient interactions in our electronic medical records (EMR) system to ensure accurate communication and continuity of care.
• Coordinate with physicians to adapt treatment plans based on evolving patient needs and feedback from caregivers.
• Educate patients and their families about health management strategies, preventive care, and lifestyle changes outside the clinical settings.
• Assist with the facilitation of hospital admissions when necessary and collaborate closely with inpatient teams to ensure safe transitions from facility to home.
• Engage in multidisciplinary team meetings, including working alongside social workers, nutritionists, pharmacologists, and other specialists to coordinate effective and comprehensive care solutions.
• Participate in kind health-related research initiatives to continually improve practice standards and patient care.
• Contribute to maintaining a welcoming, safe, and patient-focused environment in line with best practices.
• Undertake additional responsibilities as necessary to support the clinic’s mission and goals.

Patient Population / Client Focus

This role will primarily serve the elderly population within the Assisted Living Facility. Many patients will present with chronic conditions such as hypertension, diabetes, and heart disease, requiring ongoing management and support. Your ability to understand the nuances of geriatric care and to provide compassionate, informed medical attention will significantly impact their quality of life.

Qualifications

To qualify for this position, candidates must possess:

• A valid Master's degree in Nursing with specialization as a Nurse Practitioner.
• Current licensure as a Registered Professional Nurse and Nurse Practitioner in New York State.
• Board certification as a Nurse Practitioner.
• Up-to-date certifications in Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), as well as DEA and NPI registrations.
• Proficiency with electronic health records and common software programs including Microsoft Word, PowerPoint, and Excel.

Preferred Experience

We highly encourage applicants to have:

• Two years of experience in a direct patient care setting, ideally within a hospital or assisted living environment.
• Familiarity with managing heart failure, pulmonary arterial hypertension, or post-heart transplant patient populations.
• A strong background in implementing advanced therapies and medication management, particularly in outpatient settings.

Compensation and Benefits

Our compensation package offers a competitive annual salary ranging from $120,000 to $220,000, commensurate with qualifications and experience.
