Work Environment

This position is offered onsite in Albany, New York, within a supportive and collaborative work environment. You will be engaging with patients in their homes, which allows for personalized and comprehensive care. Your daily schedule will typically be from 9:00 a.m. to 6:00 p.m., with the potential for occasional evening clinics until 8:00 p.m. and participation in community outreach activities, including health fairs on weekends. This multifaceted role allows you to connect with patients in meaningful ways while balancing both your clinical responsibilities and community engagement.

Key Responsibilities

• Perform thorough health assessments and screenings, evaluating individuals and families to identify health needs and risks.
• Diagnose and manage a wide range of acute and chronic medical conditions, employing evidence-based methodologies for preventive care and health promotion.
• Coordinate with community organizations and stakeholders to develop, implement, and measure the impact of health education programs aimed at improving community health.
• Deliver culturally sensitive healthcare while advocating for patient rights and ensuring accessibility to critical health services.
• Manage referrals and follow-up care to ensure continuous and coordinated support for patients.
• Document medical records meticulously, ensuring compliance with regulatory standards and contributing to a comprehensive electronic health record system.
• Actively participate in community outreach efforts, offering health screenings and education to promote wellness within the community.
• Stay informed on emerging community health trends, public health policies, and best practices through continuous professional development and training.

Patient Population / Client Focus

The ideal candidate will work primarily with diverse populations within Albany and its surrounding communities, focusing on individuals and families who may be underserved or face challenges in accessing adequate healthcare services. You will build relationships with patients of all ages, understanding their unique backgrounds and tailoring interventions to promote optimal health outcomes.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A current, unrestricted Nurse Practitioner (APRN) license in New York State.
• DEA and NPI numbers are required.

Preferred Experience

• A minimum of 2 years of clinical experience in primary care, family medicine, or public health settings.
• Demonstrated strong clinical assessment and diagnostic skills.
• Exceptional communication and interpersonal skills, with an ability to engage effectively with diverse populations.
• Experience using electronic health record (EHR) systems proficiently.
• Ability to work independently while also thriving in a collaborative team environment.

Compensation and Benefits

The annual salary for this position ranges from USD 150,000 to USD 160,000, commensurate with experience.
