Work Environment

This full-time position is based in a collaborative school setting, providing a unique opportunity to work directly with students, families, and educators to address health-related issues. Your typical work hours will range from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics extending until 8:00 p.m. Participation in community outreach events or health fairs on weekends may occasionally be required. You can expect to work an average of 40 hours per week, ensuring a balanced approach to professional and personal life.

Key Responsibilities

• Conduct thorough health assessments and screenings for students and their families, identifying health needs and risks.
• Diagnose and manage a wide range of acute and chronic medical conditions, offering preventive care and health promotion guidance.
• Collaborate with local community organizations and stakeholders to develop and implement effective health education programs tailored to the community's needs.
• Provide culturally sensitive care, advocating for the rights of patients to access healthcare services without barriers.
• Facilitate referrals and ensure that follow-up care is coordinated to maintain continuity in patient health management.
• Maintain meticulous and up-to-date medical records to meet regulatory standards, ensuring compliance and confidentiality.
• Engage actively in community outreach initiatives and health fairs to raise awareness of available services and provide health screenings.
• Stay informed about the latest community health trends, evidence-based practices, and relevant healthcare policies that can enhance service delivery.

Patient Population / Client Focus

You will primarily serve children and families within the school environment, engaging with a diverse student population that reflects various cultural backgrounds and socioeconomic statuses. Qualifications

• Current certification as a Nurse Practitioner is required.
• Active nursing license in the state of New York.
• Master's degree in nursing from an accredited program.
• A minimum of 3 years of clinical experience, particularly in community health, public health, or school-based settings.
• Strong interpersonal and communication skills essential for fostering relationships with patients, families, and community stakeholders.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
