Key Responsibilities

• Deliver high-quality, evidence-based patient care that aligns with national and community standards.
• Collaborate with nursing and office staff to enhance office practices and operational efficiencies.
• Communicate effectively with practice management and hospital administration, representing the needs and interests of the practice.
• Ensure compliance with regulatory requirements while adjusting to evolving healthcare policies.
• Engage in patient management and provide role modeling that aligns with both practice and health system goals.
• Conduct thorough assessments of patients, develop treatment plans, and adjust care based on individual patient needs.
• Document patient encounters meticulously within 2 business days, ensuring accuracy and compliance for all medical records.
• Maintain current knowledge of industry standards and best practices to enhance patient care.

Patient Population / Client Focus

In this role, you will primarily focus on working with the elderly population residing in our Assisted Living Facility. Your responsibilities will include assessing age-specific health issues, managing chronic conditions, and developing customized care plans. Understanding the various complexities associated with geriatric patients—including mobility concerns, cognitive health, and overall wellness—will be paramount to delivering comprehensive care. You will lead efforts to establish a patient-centered practice that emphasizes communication, respect, and dignity for every resident.

Qualifications

• NYS certification and registration as a Family, Adult, or Pediatric Nurse Practitioner.
• Current AANP Board Certification.
• Valid DEA license is required.
• Ability to obtain appropriate privileges from F. F. Thompson Health.
• Familiarity with ICD10 coding is preferred.

Preferred Experience

• Previous work experience as a Nurse Practitioner is preferred, with a focus on outpatient clinical settings.
• Experience in geriatric care or internal medicine is a strong asset.
