Key Responsibilities

As a Nurse Practitioner at CareNest, your responsibilities will include:

• Conducting thorough health assessments and physical examinations to establish individualized diagnoses and develop pertinent treatment plans tailored to each resident’s needs.
• Ordering, performing, and interpreting diagnostic tests such as blood work and imaging studies to guide clinical decisions and enhance the overall treatment approach.
• Prescribing medications, implementing therapeutic procedures, and managing medication regimens to treat both acute and chronic conditions effectively.
• Providing comprehensive patient education, counseling residents and their families on health promotion strategies, disease prevention, and effective management of chronic illnesses like diabetes and hypertension.
• Collaboratively coordinating care with an interdisciplinary team to facilitate referrals, manage transitions of care, and ensure a continuity of services for optimal patient outcomes.
• Maintaining meticulous and up-to-date medical records following regulatory standards, utilizing electronic medical record (EMR) systems for documentation and communication among team members.

Patient Population / Client Focus

Your primary focus will be on elderly residents who often present with a unique set of health challenges, including mobility issues, cognitive decline, and chronic diseases. You will play a key role in advocating for their health needs while fostering their independence and enhancing their quality of life through personalized healthcare plans and constant support.

Qualifications

To be successful in this role, you will need:

• A Master's or Doctoral degree in Nursing from an accredited nursing program, ensuring a solid educational foundation in advanced nursing practices.
• Current licensure as a Nurse Practitioner in the state of New York, demonstrating your credentials and commitment to professional standards.
• Certification in a recognized Nurse Practitioner specialty area, such as family health, geriatric care, or acute care.
• Strong clinical assessment and diagnostic skills, with the ability to make sound clinical judgments in complex situations.
• Excellent communication and interpersonal skills, vital for effective collaboration with team members, residents, and their families.

Preferred Experience

Compensation and Benefits
