Sun Valley Heart and Vascular is seeking a dedicated and skilled Nurse Practitioner specializing in Cardiology to join our team in Clifton Park, New York. Work Environment

This position is based onsite at our assisted living facility in Clifton Park, offering a collaborative and supportive work atmosphere. You will be part of a multidisciplinary care team, including Physicians, fellow Nurse Practitioners, and experienced Registered Nurses, ensuring a comprehensive approach to patient care. Our facility is equipped with advanced technology for diagnostics and patient management, including tools for stress testing and electrophysiology.

Key Responsibilities

As a Nurse Practitioner in Cardiology at Sun Valley Heart and Vascular, your responsibilities will include:

• Collaborating with Cardiologists to manage a diverse patient caseload.
• Assessing and managing patients presenting with conditions such as Hypertension, High Cholesterol, Coronary Artery Disease, and Congestive Heart Failure.
• Providing specialized outpatient services in Electrophysiology for patients with abnormal electrocardiograms and arrhythmias.
• Conducting stress tests and interpreting results to assist in diagnosing cardiac conditions.
• Performing detailed patient history and physical examinations, followed by appropriate prescribing of medications.
• Working closely with the administrative support staff to streamline patient appointments and follow-up care.
• Educating patients and their families regarding cardiac health and management.
• Referring patients to specialists within our practice as necessary for enhanced care.

Patient Population / Client Focus

In this role, you will serve a diverse patient population, including individuals with chronic cardiac conditions as well as those seeking preventive care and education about heart health. The environment emphasizes personalized attention and the importance of patient education to improve long-term outcomes.

Qualifications

• A Bachelor’s Degree in Nursing with a completed accredited Nurse Practitioner program.
• Active Nurse Practitioner certification and current license to practice in New York.
• A Master’s or Doctoral degree in Nursing or related field is preferred.
• Completion of an accredited program that aligns with the specific needs of cardiology.

Preferred Experience

We welcome both experienced Nurse Practitioners and recent graduates looking to establish their careers in cardiology. Previous experience or training in outpatient cardiac care, including inpatient or outpatient cardiology, is highly desirable.

Compensation and Benefits
