Work Environment

This position operates primarily onsite with a professional, collaborative, and supportive atmosphere. You will join a team that values communication, respect, and the importance of patient-centered care. Your work schedule will typically range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Weekend participation in community outreach events or health fairs may also be required to further support our mission of improving public health. An average workweek will total approximately 40 hours.

Key Responsibilities

• Conduct comprehensive health assessments, including detailed medical history and physical examinations for patients of all ages.
• Diagnose and manage a wide range of acute and chronic conditions, while promoting preventive care strategies tailored to individual patients.
• Develop and implement personalized care plans, collaborating with patients to set and achieve their health goals.
• Engage with community organizations to create impactful health education programs that promote wellness and disease prevention.
• Provide culturally sensitive care that recognizes and respects the diverse backgrounds and needs of the population we serve.
• Facilitate referrals to specialists and coordinate follow-up appointments to ensure continuity of care, thereby enhancing patient outcomes.

Qualifications

• Active certification as a Nurse Practitioner and current New York state nursing license.

Compensation and Benefits
