Key Responsibilities

• Conduct thorough health assessments for patients, utilizing advanced diagnostic skills to identify both acute and chronic medical conditions.
• Develop, implement, and manage individualized care plans focusing on preventive care and health promotion strategies.
• Collaborate with local community organizations to design and execute health education initiatives aimed at enhancing health literacy.

Patient Population / Client Focus

This role primarily focuses on delivering care to various patient populations, including individuals and families within the Schenectady community who may experience barriers to healthcare access. You will work closely with a mix of pediatric, adult, and geriatric patients, tailoring your approach to meet unique healthcare needs. Your efforts will be integral in elevating health care equity and enhancing the overall health of our community.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited institution is required.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Active, unrestricted Nurse Practitioner license (APRN) in the state of New York.
• Possession of a valid DEA# and NPI#.
• Strong clinical assessment and diagnostic skills, proven by hands-on experience and training.
• Exceptional communication and interpersonal abilities to engage effectively with diverse populations.

Preferred Experience

• Minimum of 2 years of clinical experience, with a preference for those having a background in primary care or family medicine.
• Experience working within community health or public health settings is highly desirable.
• Demonstrated proficiency in using electronic health records (EHR) systems to document patient interactions and care plans.

Compensation and Benefits

• The competitive salary range for this position is USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
