Working onsite at our well-equipped Assisted Living Facility in Schenectady, you will be part of a collaborative healthcare team dedicated to providing the highest level of service to our residents. Key Responsibilities

• Order and interpret diagnostic tests to spearhead effective treatment plans.
• Identify and manage both acute and chronic health issues in accordance with established guidelines, ensuring optimal patient outcomes.
• Prescribe comprehensive care plans that tap into various treatment modalities, such as medications, nutrition, exercise, and traditional nursing interventions.
• Collaborate closely with an interdisciplinary healthcare team, coordinating care and making necessary referrals to specialists.
• Evaluate and document patients’ responses to treatment plans, making adjustments as needed to enhance patient care.
• Conduct telephone consultations and triage walk-in patients, ensuring they receive timely referrals to appropriate providers when necessary.
• Take lead in diagnosing and prescribing treatments based on thorough patient assessments.
• Serve as a clinical resource for healthcare staff and non-medically trained personnel, fostering a learning environment.
• Play an active role in mentoring and interviewing newly hired nurse practitioners to strengthen the team's clinical competencies.
• Contribute to the development, review, and revision of agency-specific practice guidelines for nurse practitioners.
• Engage in peer audits and quality assurance initiatives to maintain high standards in patient care delivery.
• Uphold patient confidentiality and sensitivity in all interactions, reflecting our core values.
• Directly care for patients through clinical sessions, performing comprehensive health histories and problem-specific examinations.

Patient Population / Client Focus

Qualifications

• New York State Nurse Practitioner license with certification in Family Health or Adult Health is required; an active New York State Registered Nurse license is preferred.
• Possession of a current National Provider Identifier (NPI) number and DEA certificate.
• A Master's degree in Nursing is required to apply theoretical knowledge in practice effectively.
• Experience in ambulatory care is considered an asset.

Preferred Experience

• Minimum of 3 years of relevant clinical experience is required, showcasing a breadth of skills in patient assessment and management.
• Proven ability to work collaboratively in a healthcare setting, demonstrating excellent interpersonal and communication skills.
• Experience in assisted living or similar healthcare environments would greatly enhance your candidacy.
