This role is onsite at our assisted living facility in Guilderland, New York, where our team of healthcare professionals collaborates daily to provide comprehensive care to our residents. Key Responsibilities

• Assess, diagnose, and manage acute and chronic health conditions by conducting thorough physical examinations and obtaining comprehensive health histories from residents.
• Develop and implement personalized care plans tailored to individual patient needs, which may include prescribing medications, recommending nutritional guidelines, and suggesting exercise regimens.
• Collaborate with attending physicians and other healthcare professionals in coordinating patient care, making necessary referrals to specialists or other disciplines.
• Evaluate patient responses to treatment plans and adjust care strategies accordingly, ensuring that documented outcomes reflect progress and changes in health status.
• Conduct follow-up consultations over the phone, manage walk-in patients, and provide triage to determine the appropriate level of care required.
• Support nursing staff through mentorship, assisting in the training of newly hired nurse practitioners, and contributing to the development of practice guidelines.
• Maintain accurate and confidential patient records while ensuring compliance with all health regulations and standards.
• Participate in quality assurance audits and incorporate findings into practice to enhance overall patient care quality.
• Foster a positive healthcare experience by building strong relationships with residents, families, and caregivers through effective communication and empathy.

Patient Population / Client Focus

As a Nurse Practitioner at Caring Connections, you will primarily serve the elderly population residing in our assisted living facility. Our residents often face a range of health challenges, including chronic illnesses, mobility issues, and mental health concerns. Your role will be pivotal in addressing their unique healthcare needs and enhancing their quality of life through personalized, compassionate care.

Qualifications

• Current Nurse Practitioner (NP) license in New York State, with certification in either Family Health or Adult Health. A valid New York State Registered Nurse license is preferred.
• A Bachelor's Degree in Nursing and at least three years of relevant clinical experience in a similar setting.
• Possession of a current NPI number and DEA certificate is essential for prescribing medications.
• Strong organizational skills, independent decision-making abilities, and excellent interpersonal communication skills are critical for success in this position.

Preferred Experience

• Practical experience in ambulatory care settings and knowledge of the healthcare needs of elderly patients are highly valued.
• Hands-on experience in developing care plans and managing interdisciplinary health teams.
• Prior involvement in training or mentoring healthcare professionals is an asset.

Compensation and Benefits
