Position Overview

Brightwood Orthopedic and Family Medicine Care is seeking a dedicated and compassionate Family Medicine Nurse Practitioner to join our team in Schenectady, New York. In this full-time role, you will provide exceptional, patient-centered primary care to a diverse population of adults, primarily those on Medicare. Your focus will be on delivering quality healthcare that not only treats acute conditions but also emphasizes health maintenance and disease prevention.

Work Environment

Your practice will primarily take place in an assisted living facility setting, where you will have the opportunity to interact closely with patients, staff, and family members. You will be part of a multidisciplinary team that includes a physician, nurse practitioner, physician assistant, medical assistants, and nurses, all collaborating to provide streamlined, coordinated care. Working onsite, you will have access to a dedicated medical scribe to facilitate administrative tasks, allowing you to focus on patient care. Your schedule will be Monday through Friday, from 8:00 AM to 5:00 PM.

Key Responsibilities

• Provide comprehensive primary care to patients, including conducting office visits for both routine and acute health issues.
• Administer annual wellness visits and health risk assessments, focusing on thorough and accurate documentation.
• Coordinate care with other healthcare providers, specialists, testing facilities, and community agencies to ensure the continuum of care.
• Lead initiatives related to population health, such as ensuring eligible patients receive preventative screenings.
• Engage in telephone triage and patient outreach to monitor health conditions and treatment effectiveness.
• Educate patients on their health conditions, treatment options, and care plans to enhance their understanding and engagement.
• Conduct home visits as necessary to provide care to patients with mobility challenges.
• Participate in promotional and community activities, contributing to the organization’s outreach efforts.
• Work closely with the Medical Director and operational leadership to ensure quality of care and workflow efficiency.

Patient Population / Client Focus

You will primarily serve a population of older adults, many of whom are on Medicare. Our approach focuses on delivering personalized care that respects the unique needs of each patient, promoting their autonomy and encouraging them to live fulfilling lives. By fostering trust and communication, you will build strong relationships that are essential for effective long-term care.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary Nurse Practitioner license in New York State.
• An active DEA license.
• A foundational understanding of HCC (Hierarchical Condition Categories) documentation and ICD-10 coding.
• A strong commitment to teamwork and a collaborative approach to healthcare.
• Bilingual proficiency is a plus, enhancing our ability to serve diverse populations.

Preferred Experience

• Experience working in primary care settings, including internal medicine, geriatrics, or family medicine.
• Understanding of health risk assessments (HRAs) and their role in preventive care.
• Passion for innovative healthcare models that improve population health outcomes.

Compensation and Benefits

Your expertise and commitment to excellent patient care will be rewarded with a competitive salary ranging from $115,000 to $150,000 annually, depending on your experience and qualifications. Along with a generous salary, you will receive:
