Work Environment

This position is primarily onsite at a local school setting, enhancing community involvement and accessibility for our patients. You will work within a multidisciplinary team that includes physicians, physician assistants, medical assistants, and dedicated care managers. Our support team, including registered nurses and medical scribes, ensures that you can dedicate your time to providing patient care. The outpatient practices are equipped with modern facilities and resources, enabling a conducive and efficient environment for patient interactions.

Key Responsibilities

• Deliver high-quality primary care, focusing on preventive health measures and chronic disease management.
• Conduct comprehensive office visits to address routine health concerns, acute issues, and follow-up care.
• Administer annual wellness visits and perform health risk assessments, emphasizing a holistic view of health and thorough documentation.
• Coordinate care with specialists, testing facilities, and community agencies to ensure patient needs are met.
• Lead population health initiatives, such as ensuring compliance with preventive screenings like breast cancer screenings for eligible female patients.
• Support the care team by conducting phone triage and participating in outreach efforts to maintain patient engagement.
• Educate patients and their families about medical conditions, treatment plans, and lifestyle choices.
• Assist with organizing community health promotional activities.
• Conduct home visits for patients requiring follow-up care in their living environments.
• Perform additional duties as assigned and collaborate closely with operational leadership and the Medical Director.

Patient Population / Client Focus

Our primary focus is on providing personalized and holistic primary care solutions for adults on Medicare, particularly older adults facing chronic illnesses and age-related health concerns. Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• An active DEA license.
• Direct experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and health risk assessments (HRAs).
• Strong commitment to teamwork and collaborative care.

Preferred Experience

• Previous experience in a value-based care setting focusing on older adults.
• Proficiency in the use of electronic health record (EHR) systems.
• Experience in leading health initiatives or community health programs.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
