Clinical Support Partner (EMT or MA), Street Medicine (Hemet, CA)

Clinical Support Partner (Medical Assistant or EMT) Location: Hemet, California Responsibilities Perform basic clinical care for patients experiencing homelessness, including vitals, blood draws, wound care, and EKGs. Coordinate patient care by arranging ride‑share services, picking up prescriptions, and scheduling appointments. Assist with inventory management, ordering supplies, and triaging patient phone calls and new consults. Conduct physical examinations, assessments, and develop individualized care plans in collaboration with providers and patients. Educate patients on medical issues, provide health education, and make internal and external referrals. Consult with physicians, physician assistants, nurse practitioners, and the Medical Director as needed. Maintain accurate documentation, report suspicious conduct, and comply with lab specimen and waste handling procedures. Participate in quality improvement programs, clinical committees, and ongoing training. Coach patients on managing chronic conditions to achieve health goals. Qualifications High School Diploma or GED required. Medical Assistant diploma/certificate or California EMT required. Minimum of 2 years of experience as a Medical Assistant or EMT in a medical office, field, or hospital setting. Proficient blood‑drawing skills. Knowledge of ICD‑10 coding preferred; experience with EPIC EMR preferred. Strong assessment, problem‑solving, and prioritization skills. Excellent verbal, written, and customer‑service communication skills. Ability to work culturally sensitively with a diverse patient population. Commitment to quality, safety, and infection‑control standards. Understanding of HIPAA requirements and patient confidentiality. Californian driver’s license with a clean driving record; reliable transportation and CA state liability insurance required. Additional Requirements Proof of Tuberculosis screening within the last year or valid exemption following an interactive process. Benefits and Compensation Hourly wage: $20.14–$30.55.
