• Willingness to rotate among the three outpatient locations - REQUIRED
  
• Current New Jersey license (or willing to obtain) - REQUIRED
  
• DEA and CDS - REQUIRED (for credentialing)
  
• Experience with Cerner or a comparable EMR - PREFERRED
  
• Any requested schedule restrictions - PREFERRED
  
• Any malpractice history, credentialing concerns, or restrictions on privileges - REQUIRED (at presentation)
  
• NPDB (within 30 days) - REQUIRED AT OFFER
  
• Whether the provider performs EEG interpretation, EMG, or Botox - REQUIRED
