As a vital member of our care team, you will be responsible for completing MDS assessments and ensuring accurate documentation to provide quality resident care and proper reimbursement. Key Responsibilities:

• Complete MDS assessments (MDS experience required)

• Prepare and transmit reports to Medicare & Medicaid

• Ensure accuracy of medical records and coding

• Conduct care area assessments and care planning

• Collaborate with healthcare professionals to provide high-quality patient care

Skills, Knowledge and Expertise:

• Licensed Practical Nurse (LPN) in the State of NY with MDS experience

• Strong attention to detail and accuracy

• Ability to work independently
