Key Responsibilities\n

• Serve as a crucial liaison between patients, families, and outpatient providers, ensuring clear communication and comprehensive care coordination.

• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively, ensuring patients receive the coverage and care they need.

• Participate in discharge planning and aftercare strategies, collaborating with interdisciplinary teams to ensure continuity of care.

• Provide therapeutic meal support, guiding clients toward establishing healthy relationships with food through empathetic interactions and modeling positive behaviors.

• Deliver safe, affirming, and empathetic care tailored to clients’ unique circumstances and identities, effectively addressing their emotional and psychological needs.

Patient Population / Client Focus\n

The population you will work with primarily includes older adults requiring compassionate support within a hospice center setting.
