Overview:
\n Our physician group on Long Island is seeking a SNF Physician (MD/DO) to provide clinical oversight and leadership across skilled nursing facility settings. This role combines direct patient care coordination with supervision of Advanced Practice Providers.

Position Offers:

• Competitive compensation: $275,000–325,000 per year (DOE)

Key Responsibilities:

\n

\n
• Provide primary and consultative care to geriatric residents, including oversight of complex chronic condition management
\n
• Review and manage patient charts for quality and compliance in SNF settings
\n
• Provide clinical supervision and support to midlevel providers
\n
• Communicate regularly with skilled nursing facility leadership regarding patient care coordination
\n
• Oversee clinical decision-making for geriatric patients in assigned facilities
\n
• Support continuity and coordination of care across SNF settings
\n
• Participate in team-based care planning and clinical oversight
\n

\n

Qualifications:

\n

\n
• Active, current Physician (MD/DO) licensure in New York, in good standing
\n
• Minimum 1 year of experience in skilled nursing facility (SNF) settings; geriatric care experience preferred
\n
• Strong communication skills and the ability to provide clinical supervision and oversight
\n
