Hospice Community Liaison: Referral & Outreach Expert

Three Oaks Hospice

Three Oaks Hospice is seeking a dedicated Community Liaison to promote hospice services and build referral networks within our local territory. You will engage with physicians, hospitals, nursing facilities, discharge planners, and community partners to identify admissions and educate families about hospice options. This field-based role requires 80% travel within the assigned area. You will develop and execute marketing plans, maintain professional relationships, ensure compliance with #J-18808-Ljbffr Three Oaks Hospice
