In this role, you will work directly with MPC members to address social barriers to optimal health care while providing care management through a cooperative process of assessment, planning, and facilitation. You will also act as a member advocate when referring options and community services to meet members' health needs.Maryland Care Management, Inc. Must be available and willing to travel up to 50% of the time to meet with members.Knowledge of community agencies and resources.
