Key Responsibilities\n

• Conduct thorough and personalized initial intake assessments, focusing on identifying the client's strengths, needs, and goals in a context that honors their life circumstances.

• Independently develop and implement evidence-based treatment plans with defined, measurable goals that reflect the client's unique journey and preferences.

• Maintain minimal yet essential documentation, including initial intake notes, treatment plans, and progress notes after each session, ensuring compliance with HIPAA regulations.

Patient Population / Client Focus\n

The primary focus of this role is to support individuals in hospice care, navigating the challenges that arise during end-of-life conversations and addressing complex emotional needs. Qualifications\n
