As a Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality patient care and contributing to the overall success of our skilled nursing facility.

About the Role:

As a Licensed Practical Nurse (LPN) at Nightingale at Stonegate, you will be responsible for providing direct patient care to our residents, including administering medications, performing treatments, and monitoring vital signs. You will develop and implement individualized care plans in collaboration with the interdisciplinary team, identify and address resident needs, concerns, and preferences, and promote a positive and respectful environment.

Responsibilities:

• Provide direct patient care to residents, including administering medications, performing treatments, and monitoring vital signs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Identify and address resident needs, concerns, and preferences, promoting a positive and respectful environment
• Collaborate with other healthcare professionals to ensure seamless transitions of care
• Maintain accurate and detailed records of patient care, medications, and treatment plans
• Participate in quality improvement initiatives and maintain compliance with regulatory standards
• Educate residents, families, and caregivers on care plans, medications, and health-related issues
