Ascendant Health has a team of skilled nurse case managers to navigate the healthcare system, prevent medical issues, provide choices, tailor treatments, and manage care complexities. Ascendant Health leads with innovative ideas, patient choice, great provider network, and promoting awareness of traumatic brain injury effects. Role Description This is a part-time hybrid Nurse Case Manager role located in Grand Rapids area, with some flexibility for remote work. The Nurse Case Manager will coordinate all aspects of injury-related care, advocate for their patients, and develop a care plan to manage care complexities.
