MDS Coordinator

MDS Coordinator Opportunity at Eastview Health & Rehabilitation Center

Eastview Health & Rehabilitation Center is seeking a compassionate, professional MDS Coordinator to join our team. As our MDS Coordinator, you will play a key role in assessing resident health, collecting and analyzing data, and developing individualized care plans that align with each resident's needs and expectations. This position is ideal for an experienced nurse who enjoys a combination of clinical and administrative responsibilities, with a focus on quality care and compliance.

This position is 32 hours weekly.

Job Requirements:

• Registered Nurse (RN) Preferred Open to Licensed Practical Nurse (LPN)
• Previous MDS experience in long-term care is preferred
• Strong communication, organization, and analytical skills

Job Responsibilities:

• Conduct comprehensive assessments of residents
• Develop and update resident care plans
• Monitor patient care through assessments, observation, and medical record reviews
• Collaborate with clinical teams to ensure accurate documentation and effective care delivery
• Create and manage medical codes for billing and recordkeeping
