Case Manager

The People Concern

The People Concern is seeking a on-site Case Manager for our Harm Reduction program in Santa Monica. You will manage a caseload, coordinate housing efforts, and connect clients with benefits, health services, and employment supports. This role emphasizes evidence-based, client-centered care and collaboration with an interdisciplinary team.
