Community Health Nurse Manager, COMPASS Center

We are seeking a Community Health Nurse Manager, COMPASS Center who is a registered nurse and who serves as the operational and community-facing leader for the Johns Hopkins COMPASS Center, fostering positive relationships between the institution and its surrounding communities while overseeing the implementation of community-based clinical activities (e.g., primary care clinics, health screening services, vaccination campaigns). The Manager leads a team of Community Health Workers, supports nurse practitioners delivering both in-person and telemedicine visits, and applies expertise in primary care coordination, case management, and the Epic electronic medical record to ensure safe, compliant, and patient-centered services. The Manager develops and implements community engagement strategies with local organizations, businesses, faith-based partners, and residents, and brings the business acumenbudgetary, logistical, and time-management skillsnecessary to keep community-based clinical operations solvent and sustainable.

Clinical Operations & Care Coordination

• Oversee the day-to-day implementation of community-based clinical activities, including primary care clinics, health screening services, and vaccination campaigns, traveling to 1015 clinical sites throughout Baltimore City in support of the primary care mission.
• Provide primary care coordination and case management for patients served by the Center, including referral tracking, follow-up, transitions of care, and connection to patient-level resources that address social and structural determinants of health.
• Support nurse practitioners in both in-person and telemedicine visit workflows, including visit preparation, scheduling, technology readiness for telehealth encounters, and post-visit follow-up.
• Use the Epic electronic medical record for patient registration, scheduling, documentation, in-basket management, referral coordination, and reporting; serve as the team's Epic super-user and trainer for CHW staff.
• Provide back-fill clinical and operational support in the event of staff call-outs, which may include supporting the NP in the clinical setting; clinic hours vary and may include non-standard work hours.
• Ensure clinical activities comply with Johns Hopkins Health System policies, infection prevention standards, scope-of-practice requirements, and applicable regulatory standards.

Team Leadership & Supervision

• Hire, train, schedule, supervise, and evaluate Community Health Workers who support nurse practitioners in patient registration, scheduling, check-in, and delegated monitoring duties.
• Delegate tasks appropriately within the Maryland Nurse Practice Act and monitor delegated activities performed by unlicensed assistive personnel.
• Build team capacity through onboarding, competency validation, continuing education, and performance feedback.

Community Engagement & Partnership

• Represent the Johns Hopkins Institutions to internal and external stakeholders; attend an average of 12 community meetings and/or events per week, actively engaging community residents and organization members.
• Build new and strengthen existing relationships with community and faith-based organizations; partner with community-based organizations serving the targeted community in alignment with Johns Hopkins University and Health System Outreach.
• Ensure outreach to and inclusion of community organizations in Johns Hopkins University and Health System community grant and sponsorship opportunities.
• Support the outreach and engagement efforts of the HopkinsLocal initiative with community entrepreneurs.
• Read and summarize planning documents, studies, and proposals relating to community activities; provide summaries of community meetings to the supervisor within 48 hours.

Business, Budget & Logistics

• Manage the operating budget for community-based clinical activities, including expense tracking, supply procurement, vendor coordination, and monthly variance reporting, applying the business acumen necessary to keep community-based clinical activities solvent.
• Plan and execute the logistics of mobile and community clinic operations: site selection and agreements, equipment and supply chains, staffing schedules, transportation, and event-day operations.
• Develop and manage timelines, staffing plans, and prioritization across multiple concurrent sites and campaigns, demonstrating exceptional coordination and time-management skills.
• Support the CNO in all interactions with the Johns Hopkins Health System to facilitate compliant clinical services in the community.
• Track program metrics (e.g., visit volume, screening yield, vaccination counts, referral completion) and prepare operational and quality reports for Center leadership.

Communications

• Coordinate with the Office's Communications team to promote community engagement to internal and external audiences.
• Regularly suggest and/or write stories for publication in internal and external publications (e.g., The Dome, The Hub) concerning Hopkins and community efforts.
• Publish brochures and other health education materials, ensuring health-literate, culturally appropriate content.
• Perform other duties as assigned.

Supervisory Responsibility

• Directly supervises a team of Community Health Workers (CHWs); provides operational oversight and scheduling coordination in support of nurse practitioners delivering in-person and telemedicine services.

On Call or Non-Standard Work Hour Requirements

• This position may be required to back-fill to support community-based efforts in the event of staff call-outs. This may include supporting the NP in the clinical setting; clinic hours vary and may include early morning, evening, or occasional weekend community events.

Minimum Qualifications

• Individual must be a registered nurse, licensed in the State of Maryland or state where practicing. Bachelor's Degree in Nursing or related field required.
• Minimum of three years clinical nursing experience
• Related master's degree may be considered in lieu of some experience. Prior supervisory experience may substitute for up to two years of required nursing experience.
• Additional education may substitute for required experience, and additional related experience may substitute for required education beyond a High School Diploma/graduation equivalent to the extent permitted by the JHU equivalency formula.

Preferred Qualifications

• Master's Degree in nursing (MSN), Public Health, health administration, or related field.
• Proficient use of the Epic electronic medical record (ambulatory and/or community health modules).
• Experience supervising community health workers or other unlicensed assistive personnel.
• Experience supporting telemedicine/telehealth visit workflows.
• Minimum three years related experience in community engagement, with demonstrated experience in stakeholder management, event planning and management, and managing community programs and projects.
• Demonstrated experience in budget management for clinical or community programs.
• Demonstrated experience working with the participant population, including communities affected by social and structural determinants of health.
• Bilingual (English/Spanish) skills.
• Valid driver's license and reliable transportation for travel among Baltimore City clinical sites.
• Case management certification (CCM, ACM-RN, or equivalent).

Technical Qualifications and Specialized Certifications:

• Active Maryland (or compact) RN license
• CPR/BLS certification
