LPN Profession: Licensed Practical NurseJob Title: LPN Profession: Licensed Practical Nurse Specialty: Staff Augmentation Duration: 40 hours per week Shift: Night Hours per Shift: 6:00 PM - 6:00 AM Experience: Minimum years of experience required License: Active LPN License Certifications: CPR/BLS Certification Must-Have: - Level 1 Fingerprint Clearance Card - OIG/SAM name clearance check - TB Skin Test - Hepatitis B vaccination or declination of offered vaccine - COVID vaccination or declination of offered vaccine - Agency Skills Assessment Packet Description: Schedule: 6:30 PM - 6:00 AM with options for full-time or PRN. Block scheduling which rotates with two consecutive days off and every other weekend. Administer medications and perform treatments on the assigned unit as ordered by the physician and within the scope of license. Medications may include oral, enteral, parenteral, and suppository forms. Treatments may involve sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy, jejunostomy care, and catheter care. Observe and report the effectiveness of medications and treatments. Ensure an adequate supply of medications and necessary materials. Receive and communicate information regarding the needs of residents including medical, social, and functional requirements. Participate in shift report exchanges providing continuity of care. Communicate effectively with residents, families, and staff to ensure appropriate care is provided. Complete initial admission nursing assessments on newly admitted residents. Assist in developing Minimum Data Set (MDS) assessments as well as ongoing quarterly reviews and care plans. Maintain accurate clinical and administrative documentation concerning diagnoses, treatment observations, and summaries of resident progress. Participate in Quality Assurance and Performance Improvement initiatives. Engage in staff meetings regularly. Knowledge of gerontological/geriatric nursing theory and practice. Understanding the impacts of aging and team nursing practices. Familiarity with treatment plan implementation for patient care. Knowledge of pharmacology and relevant laws, rules, and regulations governing nursing practices. Skilled in observing and reporting patient behaviors and progress. Ability to interpret laws and regulations relevant to geriatric nursing. Capable of assisting with the development of patient care plans within a team framework. Strong oral and written communication and interpersonal skills are essential. Ability to follow nursing regulations and respond effectively to medical emergencies. Evaluate and analyze client health problems and make independent decisions accordingly. Maintain effective work relationships within the healthcare environment.
