Job Description
Title: Hospice Nurse

Schedule: Days: Sunday - Thursday Hours: 10am - 7pm or 11am - 8pm

Assignment Details: Contract Weeks: 13, Guaranteed Hours: 40

Requirements:
- One year of Hospice experience.

Roles & Responsibilities:
- Admit patients to service and assess their physical, psychosocial, environmental, safety, and developmental needs.
- Develop individualized care plans based on assessments.
- Document observations and interventions related to patients' environments.
- Coordinate care with a multidisciplinary team.
- Participate in quality assessment and performance improvement programs.
- Conduct follow-up visits as needed.
